Kindergarten Registration

  • When: Begins Jan. 23, 2012 - March 30, 2012
  • Where: You must report to the Fayette County Welcome Center to register
  • Form 1: Click to download the personality form for PCES.
    This optional survey can be completed and returned to Kristin Berryman. All surveys must be returned by May 4, including new Kindergarten students to be considered in the placement process. Forms received after May 4 will not be considered. We appreciate your contribution.
  • Form 2: Click to download the Information flyer with instructions for Screening Appointments and Meet the Teacher Days. You must make an appointment for the kindergarten screening test.
  • Tours: Tours of the school will be given to parents while children are being assessed on May 3rd and 4th or at the Kindergarten program on August 9th.

All Students

 

The Fayette County Board of Education has a Welcome Center located in the Lafayette Education Building in Fayetteville. The main purpose of the center is to handle all new students' registration and all address changes of current students.

8:00 am - 4:00 pm (closed for lunch 12pm - 1pm)

 

You will need the following documents to enroll:

1. Certified birth certificate (must have imprinted, raised seal). Child must be 5 years of age on or before September 1, 2012 to enroll in Kindergarten for the 2012-2013 school year. (Please note that the certificate from the hospital is NOT an official birth certificate.) If you need to obtain one you may call 404-656-7454 or visit http://health.state.ga.us/programs/vitalrecords/Vital Records Unit for the state of Georgia. If your child was born in another state, we have the list of agencies for all states in our school office.

2. CHILD's Social Security Card.

3. One of the following documents:

Tax Bill

or

Original signed lease agreement

or

Affidavit of residency (can be obtained by contacting Deborah Davis, Lafayette Educational Center, 205 Lafayette Ave., Fayetteville, 770-460-3990 ext.122)

4. Current electric bill displaying you name and address of service.

5. Photo IDof parent or guardian registering student.

By the beginning of school the following information is required:

1. Immunization Record on the GA State Form 3231 (The Varicella vaccine and Hepatitis B vaccine are now required for attendance).

2. Eye, Ear, and Dental Record on the GA State Form 3300.

Both documents can be obtained by your pediatrician or the county health department.

 

Those moving within district or home school area:

1. Let your current home school know of the move.

2. Go to the Welcome Center in Fayetteville with your two new proofs of residence for the new address.

a. If purchasing a home - the Tax Bill
b. If renting - the lease agreement
c. A current gas or electric bill sent to you with your name and the new address on it.
